Who We Are

Platinum Community Care (PCC) was founded in 2016 with the goal of providing high quality behavioral healthcare to patients by meeting them outside of the traditional walls of their providers offices. Since then, PCC has co-located Psychiatric Nurse Practitioners, Social Workers and other physician extenders in Nursing

Since 2022, PCC has been working providing behavioral health care in the form of social workers and case managers to the NYS Hotels serving the Migrants/Asylees. In 2024, PCC began to provide Collaborative Care Management (CoCM) to Medicare recipients in conjunction with their providers.
